Thank you for selecting Orthopaedics New England for your orthopedic care. To schedule an appointment, please call us at (203) 598-0700 or use our online appointment request form.
Once you have made your initial appointment, you may preregister by downloading, printing, and completing the forms below. Doing so prior to your visit will help speed up the new patient registration process.
Please remember to bring to your appointment:
- Your completed paperwork
- Insurance cards
- A photo ID or driver’s license
- A co-pay if your insurance requires one
- Any applicable X-rays, MRIs, or other medical records
- A pharmacy card if you have one
- A list of your current medications
- Wear loose, comfortable clothing or bring a pair of shorts

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Orthopaedics New England, and they will contact us.
At Orthopaedics New England, we accept workers’ compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at (203) 598-0700 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
As a courtesy to our patients, we will bill any commercial insurance carrier assigned, but you remain financially responsible for all services furnished. Any outstanding balance after 60 days from the date of invoice is considered in default.

Workers' comp patients must bring all applicable insurance information to initial appointment including:
- Billing address and phone number
- Adjuster name and phone number
- Claim number
As of 2017, our doctors no longer accept NEW workers compensation cases but continue to follow existing patient's who are covered through workers' comp.
Surgeons at Orthopaedics New England have hospital or surgery center affiliations with Connecticut Orthopaedic Institute and Lighthouse Surgery Center.
Prescriptions and refills for established patients of Orthopaedics New England are routinely issued during our regular office hours (see above). You can make your request by calling our office at (203) 598-0700.
No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
If you have an urgent medical need and need to talk to our doctors after our regular hours, please call our office at (203) 598-0700. A doctor is on-call 24 hours a day. Your call will be returned promptly.
